Chief Medical Officer:
We are actively seeking to hire a Chief Medical Officer (CMO) with experience with the Developmental Disabled (DD) population. The CMO will be responsible for the general medical oversight and concurrent review of medical services and provide the overall clinical direction of Tri-County Care. Candidate will ensure that the medical standards are in compliance with the NYS guidelines and will evaluate developments to make recommendations for new practices and procedures. Candidate must demonstrate a commitment to our consumers welfare and well-being.

Qualification: Minimum 10 years of experience with the IDD/ DD population.

HR Director:
We are actively seeking to hire a Director of Human Resources. Responsibilities include managing recruitment, training new hires, managing performance appraisal process, benefits administration and ongoing employee relations.

The Human Resources Director will ensure that each employee within Tri County Care has the skills, support, and ability to fulfill their responsibilities in an optimal manner.

Individual must be able to multi-task and have excellent communication and organization skills.

Family Liaison:
We are is actively seeking to hire a qualified individual for the position of Family Liaison.

The Family Liaison must provide support to case management, and serve as a bridge between families and case managers, dealing with issues and/or complaints as they arise. The liaison must also conduct studies to understand patient’s needs, and advocate on their behalf in a timely and efficient manner.
A minimum of two years of practical experience with the DD (Developmental Disabled) population, is required, as well as excellent verbal and communication skills.

Training Specialist:
We are actively seeking to hire an experienced Training and Education specialist to join our team.

The candidate will be responsible to develop and deliver training on topics and processes related to CCO’s and the DD (developmental disabled) population. This involves creating a curriculum including training programs and initiatives and evaluating outcomes to enhance the training program. The specialist must provide comprehensive training to new hires and existing employees on an ongoing basis.

The Training and Education specialist will ensure that each employee within Tri County Care has the skills to excel in their current role and is prepared for future responsibilities.

Regional Directors:
Tri County Care, a newly formed Care Coordination Organization, (CCO) is seeking several individuals for the position of Regional Directors of Care Management in the New York City areas. This position will oversee the operations of the geographic area and support and promote the mission and values of TCC .

The Regional Director is responsible for the direct support for the care manager directors and oversee that all care coordination activities are delivered to satisfaction. The regional director will also support all operational staff in their region, while helping to create an environment and culture that promotes the values of TCC. The position is also responsible for ensuring compliance with all state and federal regulations, as well as organizational policies.

The Regional Director will lead the department to ensure that individuals and families are at the center of all we do and that we strongly advocate and protect individuals’ rights.


• Bachelor’s degree in Human Services or related field is required. Master’s Degree or licensed professional preferred.
• A Minimum of ten years’ related IDD work experience is required with 3 years previous management at a senior management level.

Responsibilities include ensuring that the individual is a good fit for the CCO, and conducting/overseeing the process of screening and admissions. Candidate will also be responsible for the organizations outreach and networking activities within the Developmental Disabled (DD) populationwhich includes contributing ideas and leads for identifying new referral sources and providing the necessary follow ups.

All intake and outreach activities must be conducted in compliance with NYS regulations. Minimum of 5 years’ experience with the DD population required.
