Tri-County Care is privileged to serve thousands of individuals throughout New York State, empowering people with developmental disabilities to lead a full and meaningful life of their choosing. Through holistic, comprehensive, and culturally sensitive care management, we serve as lifetime care advocates for people with I-DD and their families.

Our diverse spectrum of provider agencies span all five boroughs of New York City, Long Island as well as throughout the Hudson Valley and Capital regions. They represent the rich mosaic that New York is famous for, and we are proud to partner with them to provide care and support for thousands of individuals from many different ethnicities, languages and cultures.

We’re proud that Tri-County Care, sponsored by the New York State Hamaspik Association (NYSHA), has recently been designated as one of 7 Care Coordination Organizations (CCO) serving New York State.

CCO are part of a new and improved system of care that allows people with I-DD to plan all their needs in one place—including disability services, community-based supports, and health and wellness services—rather than having to coordinate with several agencies to access services.

For you, CCO mean more holistic, comprehensive care, more options, more flexibility, and fewer barriers to access. They also ensure that everyone involved in your care is aligned with your personal goals, wants, and needs.

Yoel Bernath
Chief Executive Officer

Yoel has established himself as a leader in the health and human services industry for close to 20 years. Beginning his career at an entry level position at a nonprofit agency serving the OPWDD population, he has worked his way up to senior leadership positions, including opening and operating HamaspikCare, a Licensed Home Care Services Agency serving 13 counties throughout NYS. In 2013, under his leadership and direction, the Hamaspik Choice MLTC Plan began operations throughout the entire Hudson Valley region. In his role as the Executive Director of Hamaspik Choice, Yoel and his team have worked to cultivate relationships with community partners and build a strong network of service providers. Yoel was appointed as the Executive Director for Tri-County Care CCO. His unique experiences with both the OPWDD as well as MLTC populations enable him to provide perspective on this Care Coordination Organization. Yoel Bernath is leading the launch of HAMASPIK, Inc HMO which is expected to go live in 2019.

 

Jackie Spring
VP of Operations

As Deputy Director for Community Services, Jackie is responsible for overseeing/ managing operations. Prior to her current role, Jackie worked for more than 40 years at Hudson Valley DDRO in various leadership capacities. She has a degree in Social Work from Fordham University.

Nechama Nissenbaum
VP of Business Growth and Development

Nechama is a Registered Nurse who brings 18 years of dedicated experience and a passion for  servicing individuals with I/DD. Prior to her current role, Nechama served as an RN responsible for individuals residing in group homes. Nechama’s experience also includes MSC Supervisor, Early Intervention Coordinator, TBI and NHTD Manager.

Blimi Kornbluh
VP of Human Resources

Blimi brings extensive business administration skills and experience to her role as VP of Human Resources. Before assuming her role, Blimi served as the HR coordinator for HamaspikCare and Hamaspik Choice MLTCP for close to a decade. Blimi currently handles all aspects of HR at TCC and is devoted to establishing and maintaining a satisfied workforce.

Mendy Ornstein
VP of Finance

In his current role as VP of Finance, Mendy manages all financial operations in Tri-Count Care. He brings 7 years of finance experience in the healthcare industry.

Japhe Deliat
Director of Training and Education.

Japhe joined team TCC as a Training Specialist in August of 2018 and now serves as the Director of Training & Education. Since 2001, Japhne has been working with the I/DD population, serving as Direct Support Professional in Residential Programs, Community Habilitation and Respite. Japhe has since assumed the roles of Assistant Director of a Day Program, Senior Residence Manager, and Residence Director. He loves to empower employees to do better at their job and being more passionate about what they do.

Jason Mazzuca
Regional Director

Jason brings 26 years of experience in the field of servicing individuals with developmental disabilities. He has served as Residential and Day Program Manager, Director of Quality Assurance (a position he held for 16+ years), and is currently Director of MSC for Tri County Care. Jason joined TCC for the chance to be aligned with an agency that shares his values and commitment to providing high quality services for individuals with I/DD.

Fatima Nunez
Regional Director

Fatima Nunez is a downstate Regional Director at TCC. She has close to two decades experience in the field of Intellectual/Developmental Disabilities and Traumatic Brain Injury in varying leadership and management capacities, ranging from program management, compliance and education. Fatima has a Bachelor’s in Psychology and a Master’s in Education.

 

Contessa Officer
Regional Director

Contessa has dedicated 18 years of service to individuals with developmental disabilities and their families. Having worn many hats, Contessa started her career teaching students on the Autism Spectrum in Los Angeles at the Help Group. She served as the Clinical Coordinator supervisor for Community Habilitation services at Eden II and, prior to coming to Tri-County Care, she was the Consumer Advisory Board (CAB) Regional Director for Long Island, Brooklyn and Queens, serving the Willowbrook class.

Michael Wright
Regional Director

Michael has been in the field of supporting people for over 20 years and has dedicated his life to the service of those in need. In his role as Regional Director Michael is responsible for overseeing the TCC Care Managers in his region to ensure individuals are provided the highest quality services. Prior to his current role, Michael has worked in every area of service delivery for people with I/DD. He joined Tri-County Care because its philosophy of service delivery is meaningful and progressive.

Perry Zelik
Regional Director

Harboring a passion from a young age for helping families and children with special needs, Perry started out volunteering as a respite worker as a teen. She moved on as a Medicaid Service Coordinator for 3 years and ultimately advanced to MSC supervisor with Hamaspik of Orange county for over 15 years. Currently, Perry directs the TCC Care Managers in her region to ensure individuals with disabilities are provided the highest quality services.
