I was concerned that my 15 year old had been refusing to go to school for several weeks since his paraprofessional resigned. As soon as we switched to TCC, they assigned me a new Case Manager, who immediately sprung into action. He scheduled a meeting at the school with my child, the school administration and me. He worked with us to reintegrate my son into school. He also set up behavioral health & psychiatric counseling to help my son manage his anxiety. We worked together to finalize his Life Plan. I am so grateful for the excellent CM that TCC connected us with, and I’m happy to report that my son is back at school and doing great!
I suffer from anxiety and phobias, so it’s very difficult for me to trust others, particularly new people. Nevertheless, my care manager was so kind and empathetic that it was easy for me to develop a very good relationship with her and move forward. She worked tirelessly to help me get my Social Security benefits reinstated, arranged countless phone calls and meetings on my behalf, and went with me to personally attend the many appointments that were assigned. All if this was done with the utmost patience and sensitivity, and after many months of great effort, we saw concrete results–my benefits were reinstated to the delight of my family and me!
My son is 14 years old and has Cerebral Palsy. Because of his diagnosis he walks with difficulty and needs assistance with his ADLs. Previously, our bathroom did not accommodate his needs and he was forced to rely on me to assist with bathing him. TCC’s care manager worked very hard to help us modify the bathroom through the EMOD program through OPWDD and ultimately succeeded. Now, my son is able to use the bathroom and shower independently because the tiles have been switched to non-slip tiles and the shower is a roll-in shower with easy accessibility. He is so appreciative to gain independence! I am extremely grateful to TCC for providing the assistance we so urgently needed.
Because I have low muscle tone, I use a wheelchair and an adaptive stroller for transportation. However, my old stroller was not supporting me correctly and a newer model was not approved to be funded by Medicaid. My Care Manager worked diligently with the EMOD/AE program at OPWDD to acquire funds the necessary funds and thanks to her the request was approved. I now enjoy the comfort and safety of an adaptive stroller that provides the support I need. Thank you TCC!
I have Autism and I was struggling in a school system that was not meeting my needs, both academically and socially. Together with my advocates, my TCC care manager arranged a new specialized school placement for me and also acquired the funding needed for it. She even came up with a creative solution for a transportation issue that I was dealing with. I am so much happier today, thanks to the dedicated efforts of my care manager! I feel that I am finally thriving in my new school, and I love the programming and staff.
